STATEMENT OF CONSENT TO RELEASE CONFIDENTIAL INFORMATION

Republic Act No. 10173 entitled “An Act Protecting Individual Personal Information in Information and Communications Systems in the Government and the Private Sector, Creating for this Purpose a National Privacy Commission, and for Other Purposes”, or simply, Data Privacy Act of 2012 (DPA), is the law that gives form to the declared policy of the State to protect the fundamental human right of privacy and communication.

The University of Science and Technology of Southern Philippines (USTP), is committed to uphold, respect and value data privacy rights in compliance with the Data Privacy Act of 2012 (DPA), its Implementing Rules and Regulations (IRR), and other relevant policies including issuances of the National Privacy Commission (NPC).

I understand that, in accordance with the Data Privacy Act of 2012, its IRR, other relevant issuances of the NPC and the USTP Data Privacy Manual, the USTP Career Center will not release or disclose non-directory information to third parties, except those listed in the Placement Partners Program. I hereby request, give my permission and direct the Career Center to discuss with and release the specified portions of my resume database to prospective employers.

I understand that the purpose of the release or disclosure of my resume database is for employment, subject to the requirements of the applicable laws, regulations, or ethical standards. Client and employer information too from the website may be used to construct aggregate reports that assist the USTP Career Center  in program planning for statistical reports or to inform clients of services and programs of interest. This information may also be used to troubleshoot problems and is accessible only to USTP Career Center staff. Information routinely shared with parties outside of the USTP Career Center is shared only in aggregate form.

I understand that in order to ensure my privacy is maintained, USTP-Career Center will be verifying the identity of the person to whom access to my resume database may be provided. Only employers who are in the Career Center Placement Program will be given access of the résumé databank provided that they will sign for a Non-Disclosure Agreement. They shall ensure to operate and hold of the data under strict confidentiality and not to intend for public disclosure. This obligation shall continue even after leaving the their company, transferring to another position, or upon terminating their employment or contractual relations.

Authentication: Programs that require registration utilize a login name and password to authenticate returning users.

I understand that 1) I have privacy rights as outlines in R.A. 10173 (Data Privacy Act) and USTP Data Privacy Manual and 2) this consent will be effective until I submit a written revocation to the Career Center.

I have read, understand and voluntary give consent to the USTP Career Center to collect my personal information for my resume database and disclose to a third party with the contention that is processed in adherence to the general principles of transparency, legitimate purpose and proportionality of the DPA.

CONFIDENTIAL DISCLOSURE AGREEMENT

The University of Science and Technology of Southern Philippines (USTP) protects the confidentiality of student personal information and the individual student’s right to privacy, in compliance with the Data Privacy Act of 2012 (DPA), its Implementing Rules and Regulations (IRR), and other relevant policies including issuances of the National Privacy Commission (NPC).

As a member of the USTP Career Center Placement Partner, you have a responsibility to respect the confidentiality of the student personal information you may be given access to. This means you are required to conduct yourself in a professional manner when it comes to handling the student personal data and ensuring that the information is not collected, stored, maintained or disseminated in violation to the data privacy laws in the Philippines.

Please read and agree to the guidelines outlined below regarding the appropriate use of student personal information made available to you:

  • I understand it is my responsibility to comply with the Data Privacy Act of 2012 (DPA), its Implementing Rules and Regulations (IRR), and other relevant policies including issuances of the National Privacy Commission (NPC).
  • I understand that I may be granted access to student personal information maintained by USTP Career Center and this information may be made available to me on paper, contained in program software, visible on screen displays, in computer readable form or in another format.
  • I understand that this information is to be used by me with strict confidentiality for the sole purpose of evaluation for potential applicant in employment and is not to be discussed, copied or shared with others nor intended for public disclosure. This obligation shall continue even after leaving my employment, transferring to another position, or upon terminating my employment or contractual relations.
  • I understand that passwords I create for access to approved system will not be shared with anyone, and I will securely log in and out of programs that store student specific data.
  • I understand student information should not be left unattended or unsecured at any time where others could access the information without my knowledge.
  • I understand student personal information should never be transmitted via email or as an email attachment to others other than myself as a member of the USTP Career Center Placement Partner.
